I realize that sometimes filling out forms right before your massage can be stressful in itself. For your convenience, I have included my intake form below for you to print and fill out at home or at your office. But please be sure to bring it along and hand it to me before your first session. Otherwise, you will need to fill one out before we begin. Please note that you will need Adobe Acrobat Reader to open the file. You can download the latest version here. Confidential Client Information and Health History Massage information and frequently asked questions. Massage The application of soft-tissue manipulation techniques to the body, generally intended to reduce stress and fatigue while improving circulation. Swedish Massage Swedish massage is a gentle, rhythmic massage administered to relax soft body tissue and facilitate healing. It is lighter in touch than other forms of massage and includes long flowing strokes, kneading, and friction techniques on more superficial layers of the muscles. It can loosen stiff joints, reduce muscle tension, and help clear nasal or chest congestion. Swedish massage contributes to overall well-being and is widely prescribed by physicians to enhance treatment for insomnia, anxiety, and depression. Deep Tissue Massage Deep tissue massage focuses on the deeper layers of muscle tissue. When muscles are stressed, they block oxygen and nutrients, leading to inflammation that builds up toxins in the muscle tissue. A deep-tissue massage helps loosen muscle tissues, release toxins from muscles and get blood and oxygen circulating properly. This type of massage also breaks down scar tissue, known as restrictive fascia (pronounced, fasha). Clients are usually instructed to drink plenty of water for a couple of days following the massage to help eliminate the released toxins. Sports Massage Sports Massage is a type of massage designed for highly active people who engage in athletics. Engaging in sports is harsh on the body and can often lead to injuries in both the short and long term. Sports Massage enhances performance and prolongs a sports career by helping to prevent injury, reduce pains and swelling in the body, relax the mind, increase flexibility, and dramatically improve recovery rates. Sports Massage is also highly effective in aiding the rapid recovery of an athlete from an injury by encouraging greater kinesthetic awareness and in turn promoting the body’s natural immune function. Benefits of massage • Relieves stress and promotes an overall feeling of relaxation, helping you breathe and move more easily. • Alleviates pain and tension, and helps you manage pain better. • Decreases inflammation. • Promotes speedy recovery from injuries (work, trauma, accident or sports-related). • Increases mobility and provides greater joint flexibility and range of motion. • Improves circulation. • Improves and strengthens immune system. • Relieves musculoskeletal problems and improves posture. • Relieves pregnancy discomforts. • Reduces anxiety and promotes an overall feeling of well-being. • Increases body awareness. How should I prepare for my massage? I recommend that you come dressed comfortably and take no aspirin, pain killers, or alcohol on the day of the appointment. To receive bodywork, you can dress down to your comfort level; however, the more clothing you have on, the less soft tissue I will be able to affect. Draping options include covering by a sheet or towel. Where Will My Massage Session Take Place? Your massage session will take place in a warm, comfortable, quiet room. Soft music will be played to help you relax. You will lie on a table especially designed for your comfort. What Should I Do During the Massage Session? Make yourself comfortable. I will either gently move you or tell you what is needed throughout the session (such as lifting your arm). Many people just close their eyes and completely relax. Others like to talk during their session. Feel free to ask questions about your massage. How Will I Feel After the Massage Session? Most people feel very relaxed. Some experience freedom from long-term aches and pains developed from tension or repetitive activity. After an initial period of feeling slowed down, people often experience increased energy, heightened awareness, and greater productivity which can last for days. It is recommended that you drink plenty of water following your massage to help eliminate the released toxins. |
